00:00:07: Retention has always been one of orthodontics' biggest puzzles with biological, mechanical & behavioural factors all playing a role.

00:00:17: In this episode we'll piece together the scientific rationale behind orthodotic retention.

00:00:31: When I think about orthodontic retention, I am always reminded of Oppenheim's old remark.

00:00:37: Retention isn't a problem it is the problem.

00:00:41: in this review Declan Millett explores why despite decades of research we still struggle to keep teeth where We've placed them.

00:00:49: he shows that relapse Is not simply the result Of poor compliance but The expression of biology growth and time.

00:00:57: After tooth movement, the periodontal and gingival tissues are stretched and disturbed.

00:01:03: They need months to reorganize And their elastic fibers continually pull toward original position.

00:01:11: The alveolar bone remodels slowly.

00:01:14: That lack creates biological instability.

00:01:18: Then we have this soft tissue envelope The balance between lips, cheeks & tongue.

00:01:23: Teeth live in a neutral zone of pressure.

00:01:26: If we push them beyond that zone, especially in the lower labial segment relapse is inevitable.

00:01:33: The original lower arch form must be respected.

00:01:37: expansion particularly transverse expansion remains the least stable movement We make.

00:01:43: millet also reminds us that facial growth and aging continue long after active treatment ends.

00:01:49: Mandibular rotation and soft tissue adaptation can create late-lower incisor crowding even in untreated individuals.

00:01:57: It's a natural process, not a failure of orthodontics!

00:02:01: To reduce relapse, adjunctive measures like circumferential supracrestophiberotomy or interproximal reduction may help but they do NOT eliminate the problem.

00:02:11: Pharmacological methods are still experimental... ...and so we arrive at the paradox of modern retention.

00:02:19: We prescribe lifelong retainers for everyone, yet we know that change will still occur.

00:02:24: The clinician cannot hold back craniofacial aging indefinitely.

00:02:29: What we can do is plan intelligently tailor retention individually and involve our patients in every decision.

00:02:38: In the end this review leaves us with humility.

00:02:41: Stability is not a guarantee.

00:02:42: it's dynamic balance between tissue biology growth And patient behavior.

00:02:48: Our task is to guide that balance not to pretend we can freeze it in time.
