Key Findings:

  • Stainless Steel Twist-Flex: Did not fail, highest load capacity (445 N).
  • Titanium Grade 5: Only CAD-CAM option matching strength.
  • Others (Nickel-Titanium, Zirconia, Gold, Cobalt-Chromium, P.E.K.): Premature breakage or debonding.

Conclusion: Stainless steel remains the gold standard for long-term retention.

00:00:00: Today, we're testing six modern CAD-CAM retainers against the classic stainless steel version to see which one lasts longer and holds teeth more stable.

00:00:11: Let's find out whether technology has truly replaced tradition when it comes to orthodontic retention!

00:00:31: The researcher simulated fifteen years of wear, one point two million chewing cycles under standardized conditions and then tested how much force each retainer could withstand before failure.

00:00:44: The results were striking!

00:00:46: The conventional stainless steel twist flex did not fail once.

00:00:51: it also showed the highest load capacity around four hundred forty five newtons which is well above normal incisal bite forces.

00:01:01: Among all CAD-CAM options, only titanium grade five retainers survived the entire test and reached comparable strength levels.

00:01:12: Every other digital retainer type – nickel titanium, zirconia gold cobalt chromium & PEK showed either premature breakage or debonding.

00:01:24: especially PEK failed completely Neatly fractured frequently, and because of its elasticity those fractures could remain invisible until relapse occurs.

00:01:36: From a clinical point-of view this means that twist flex remains the gold standard.

00:01:42: Titanium grade the fifth may become good alternative but others are not mechanically sufficient for long term use.

00:01:50: The authors model was highly standardized and realistic, but we must remember it is still an in vitro simulation.

00:01:57: Real mouths are more complex!

00:02:00: So until further long term clinical data exist I would advise staying with what works.

00:02:07: a hand-bend multi stranded steel retainer properly bonded and monitored continues to offer the most reliable long-term retention we have.
